Importation requirements for poultry include:

1. An original health certificate issued by an accredited veterinarian within 30 days prior to entry. Photocopies of health certificates must have an original veterinarian signature.

2. There are no other specific entry requirements providing the poultry are healthy and do not originate from a poultry quarantine area.

3. A National Poultry Improvement Plan Form 9-3 (domestic poultry only).
